Abstract Objective To evaluate the misdiagnosed profile for acute myocardial infarction(AMI), pulmonary embolism(PE)and aortic dissection(AD)in China using big data firstly, andprovide reference evidence for improving the diagnostic level. Methods The misdiagnosis literature and its prevalence and scope of AD, PE and AMI in the misdiagnosis literature database from 2004 to 2013 were included, and the diagnostic thinking about acute chest pain also discussed. Results There were 1514 pieces of misdiagnosis literature for AMI, PE and AD totally, and cumulative misdiagnosed cases 19 238 cases, and misdiagnosis rate was 19.62%(AMI), 53.00%(PE), and 38.58%(AD)separately. The misdiagnosed diseases spectrum was related nearly a hundred kinds of diseases, and involved more than 10 medical specialties. The AMI patients misdiagnosed as digestive system diseaseswere topped with the most frequent disease as acute gastroenteritis, the PE patients misdiagnosed as respiratory system disease topped with the most frequent disease as acute coronary syndrome; and the AD patients misdiagnosed as other circulatory system diseases topped with the most frequent disease also as acute coronary syndrome. Conclusion Emergency physicians should establish horizontal and three-di⁃ mensional thinking method, to analyze the etiology distribution from the macro, overall level,especial⁃ ly to pay more attention to capture a sign of cardiovascular emergencies from acute gastrointestinal symp⁃ toms and manifestations of the central nerve system was the important thinking method to avoid the mis⁃ diagnosis for the triad of acute chest pain.
